New Patient Forms

To help us understand your needs and make your first visit as smooth and efficient as possible, we invite you to complete the patient information form prior to your appointment. Providing this information in advance allows our team to prepare for your visit and helps us expedite the appointment process.

Appointment Guidelines

Your appointment time is reserved exclusively for you. Our team thoughtfully prepares for each visit so we can welcome you warmly, give you our undivided attention, and provide the highest level of care.

If you need to reschedule or cancel your appointment, we kindly ask that you provide at least 48 hours notice. This gives us the opportunity to offer the time to another patient in need of appointment and helps you avoid a last-minute cancellation fee.

As a courtesy, we will send appointment reminders before your visit. We also encourage you to keep track of your scheduled appointment and allow plenty of time to arrive promptly, so your visit can begin as smoothly as possible.

Insurance

We are a fee-for-service practice. Payment is due at the time of treatment, and we do everything possible to help you receive the full benefits of your insurance policy, including preparing and filing claims to your carrier for your reimbursement.

Dental insurance works differently than medical insurance, so it helps to know the following: your policy is a contract between you, your employer, and your insurance carrier, and our office is not a party to that contract, so we cannot guarantee estimated coverage or payment. Insurance companies base payments on their own fee schedules, and not every service necessary for excellent dental health is a covered benefit. Elective cosmetic procedures are generally not covered, though when a cosmetic treatment serves a restorative purpose, we will submit case notes, photographs, and X-rays to your carrier to support coverage.

Visiting Our Office

Our state-of-the-art office sits at the edge of Georgetown in Washington, DC’s West End, at the corner of 25th and M Streets, three blocks from Foggy Bottom Metro. We’re an easy trip from Downtown, Dupont Circle, K Street, and Arlington.

Parking is available in the building; the entrance is on M Street between 24th and 25th. Our office is on the third floor in Suite 328.
